What you can do:Book the call whenever you're ready. Your loved one doesn't have to be on this first call.

Your free intro call is a chance to talk through what you've been noticing and ask any questions you may have. By the end of the call, you'll have a clearer sense of what kind of evaluation or support might make sense for your loved one.

What Sunday Health does:Listen to your concerns, help you understand your options, and determine whether you or your loved one are a good fit for Sunday Health.

You and your loved one will be asked to complete online intake forms and a pre-appointment assessment ahead of your visit. For the most helpful and efficient experience, please complete them as soon as possible, and at least 24 hours before your appointment.

What Sunday Health does:Review your information in advance to ensure your visit is as focused and effective as possible.

What you can do:Check your email and complete the forms at least 24 hours before your appointment.

What happens during the initial visit: Your clinician will start by building a complete picture, including medical history, medications, family history, and any changes you’ve noticed. They’ll also guide you through cognitive assessments, such as questions, memory exercises, and brief tests, to better understand thinking, memory, and behavior.

If additional testing is needed: Your clinician may recommend further testing, such as blood work, brain imaging, or other diagnostic tools. Not everyone needs additional testing, but when it’s helpful, we’ll explain exactly what it’s for and what it can tell us.

What Sunday Health does:We provide clear, thorough insights so you understand your results and what they mean for you and your family.

What you can do:Write down your questions ahead of time. There are no wrong ones.

After your evaluation is complete, you’ll meet with your clinician to go over everything together. This includes your cognitive assessment results, as well as any labs, imaging, or additional testing that was completed to build a comprehensive understanding of your brain health. If appropriate, your clinician may also discuss whether a diagnosis is indicated.

You’ll talk through your individual risk factors and what they mean for your overall cognitive health. Together, you’ll develop a clear, actionable care plan focused on addressing modifiable factors that may help prevent, delay, or slow cognitive decline.

This visit is also a dedicated time for you to ask questions and make sure you fully understand your results and next steps. You’ll leave with a plan you feel confident moving forward with.

What Sunday Health does:We bring together all aspects of your evaluation to provide clear answers and a personalized care plan tailored to your needs.

What you can do:Bring any questions you have about your results or next steps. Consider including a family member or care partner to help support you in understanding and following the plan.

After your evaluation, your care team will create a personalized brain health plan based on your results, goals, and risk factors. This may include lifestyle guidance to support memory and thinking, such as sleep, nutrition, and daily habits, as well as medication review and management if appropriate.

As your needs evolve, your team will continue to monitor changes over time and adjust your care plan. If treatment is needed, we’ll help you understand your options, including access to the latest therapies, and guide you through next steps.

You’ll also have ongoing access to your care team between appointments for questions, support, and coordination of care, including connections to local resources or clinical trials when appropriate.

What you can do:Continue to support your loved one by staying engaged, asking questions, and communicating any changes or concerns with your care team.
